How it handles tricky CSV

Follows RFC 4180: fields in double quotes may contain commas, line breaks and doubled quotes ("").
Strips the UTF-8 byte order mark that Excel adds, so the first header is not broken.
Rows shorter than the header get empty values; extra values go into _extra.
